Provide Multi-monitor screen preflet support for Radeon HD driver. Test adapter: Radeon HD 4350/4550 (RV710, 1002:954f)

Test adapter has 512MB VRAM. During radeon_hd_init: Mapped a frame buffer of 256MB out of 512MB VRAM.
