See attached screen-shot.
Menu -> Demos -> Cortex.
Click on 'HD Audio' and then play around with combinations of Start/Stop on the Transport window, and various of the Media add-ons Window.

This is on a build-from-source of hrev53658 on a Dell D10D https://www.cnet.com/products/dell-wyse-d10d-thin-client-g-t48e-1-4-ghz-monitor-none-series/

Got a definite reproduction:
- In the Cortex window, click HD-Audio
- In Transport, click Start then Stop
- In the Media add-ons, double-click AudioAdaptor
- In Transport, click Start then Stop
- In the Media add-ons, double-click Audio Mixer
That reproduces the problem on my system.
