Better ProcessController - Debugger integration

Description

  1. Currently ProcessController allows to debug a thread. When choosing that option the debug server's crash dialog opens in which one does again have to choose to debug the thread. It would be nicer, if PC would directly open Debugger.
  2. PC should also allow to debug a team. That more or less makes Debugger's teams window superfluous (though it certainly doesn't harm to keep it).
