intel_extreme regression: app_server crash on boot-up

Description

This is hrev56004, 64bit.

After an update to hrev56004, the app_server crashes when booting up (see attached syslog).
I can boot into Haiku using fail-safe graphics. I imagine it's to do with the recent intel_extreme changes?

KERN: intel_extreme: intel_extreme_init: completed successfully!
KERN: intel_extreme: CALLED status_t device_close(void*)

The device initialized succesfully but was then closed (probably the accelerant did so.) Do you have a stale driver or accelerant in your "non-packaged" directories?

Otherwise the crash looks like the standard "accelerant failed init" crash. (We really should fix that.)
